StartDate: 3/23/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Post-discharge planning and provide resources for post-discharge; Care Coordination, Continuum of Care, Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – WA/compact RN license; ADN or BSN; 2 years of recent acute inpatient case management experience; 4 years of overall nursing experience; Travel experience; Epic experience; Healthcare financial and reimbursement knowledge

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Sun/Mon/Tue/Fri: 7am-5:30pm

SYSTEMS – Epic
